Baptism and/or Confirmation
You have been given the great privilege to lead a child to be baptized in their Catholic Faith! Or you have been asked by a young Catholic to be their Sponsor in Confirmation, supporting them in their relationship with Christ!
Here is the form that the Archdiocese of Hartford asks those asked to be sponsor sign and bring to the respective parish. In the form the person attest that he/she has received the sacrament, if married, married sacramentally. Most importantly, the person attests that he/she is practicing the Catholic Faith, specially partaking of the Holy Eucharist on Sundays.
